Measurement of Stress, Anxiety and Depression in the Patients with Tinnitus and Their Clinical Significance

Abstract
Background and Objectives : According to the neurophysiologic model of tinnitus, emotion and autonomic nervous systems are closely related to generation of tinnitus which may suggest the clinical relationship between tinnitus and psychological status such as stress, depression and anxiety. We performed this study to measure psychological status in the patients with tinnitus using standardized measurement tools and to consider its results for therapeutic clinical application.

Materials and Methods : One hundred and four tinnitus patients who were diagnosed as sensorineural tinnitus through audiologic evaluation were included in this study. Tinnitus and psychological status were measured by tinnitus questionnaire, Korean version of BEPSI (Brief Encounter Psychosocial Instrument) scale, Beck Depression Inventory, Spielberger State Trait Anxiety Inventory). Scores of each psychological assessment were compared with those of 98 controls.

Results: Stress, depression and anxiety scores were significantly higher in the patients with tinnitus than control subjects. Depression and anxiety scores were higher especially at the early stage of tinnitus development. Significant negative correlation was also observed between age and anxiety. There were no significant correlations between subjective scale of tinnitus/psychoacoustic measurement of tinnitus and psychological status scale.

Conclusions: Status of stress, depression and anxiety which might be much higher in the patients with tinnitus than normal person should be assessed for understanding individual psychological status of the patients and its application to their counseling and therapeutic approach.
